Please update your child’s medical information and consents on Power School so the nurse can care for your child if needed when they are at school.
Directions: Log onto Power school, on the left, click on “School Information”, scroll to the bottom and click on “School Forms”, Under your child’s name click on “Medical”, update “Student Health History” (add information the nurse needs to know then scroll to the bottom, type your name and select “Save Form”, select “Yes” if you are okay with the nurse caring for your child, select “Yes” if you want the nurse to do vision and hearing screenings, select “Yes” if you want your child to receive Acetaminophen/Tylenol at school. If you leave it blank, it translates to “No”.

Meet our Community School Liaison
Hi! My name is Melanie Morales and I am the new Community Schools Liaison here at Merdian Elementary School. I have been a part of the West Ada community for about 5 years and I'm so excited to now be a part of the Meridian Elementary family!
As a Community School Liaison, I work closely with our school counselor, and our social worker, to help connect our families to much needed resources, information, and assistance that will in turn allow our students to have the best opportunity at reaching their educational success goals.
I am so passionate about making sure each of our kids gets the same opportunity at success and I am grateful to be able to help support that in any way possible. I'm so excited for this new change for Meridian Elementary, and our community, and to get to know all of you!
Bus Information
Prior to the start of the school year, we kindly request that you take the following steps to ensure that your student is assigned a transportation route:
1.) Login to PowerSchool
2.) If your child has a route assigned to them - GREAT!
Please review the route assigned as well as your stop locations and times.
