2022-2023 Registration & Back to School Information
- Teacherease will be open from Wednesday, July 20th - Wednesday, August 3rd. All families, new and returning, need to complete registration. www.teacherease.com
- Any 2022-2023 kindergarten student who has completed their kindergarten screening with Mrs. Kummer is considered a returning student - not a new student.
- New students would be students who are transferring into the district, and any kindergarten students who have not yet completed their Waltham screening.
Helpful Information for Parents:
Proof of Residency Requirement for Families New to Waltham
Every family must show 3 proofs of residency before completing the registration process. Proof of residency is required for all new families to the district.
New families will need to bring one (1) of the following documents with them: recent property tax bill and proof of payment, mortgage papers, signed and dated lease and proof of last month's payment, or letter from manager and proof of last month's payment.
In addition, new families must also need to bring in two (2) of the following documents with them: current driver's license, vehicle registration, voter registration, most recent cable or credit card bill, current public aid card, current homeowners or renters insurance policy and premium payment receipt, most recent gas, electric, and/or water bill, current library card, receipt for moving truck rental, or mail received at new residence.
Fees
Registration fees per student:
$90 registration,
$30 technology,
$6.50 tote bag fee for new students in grades K-4,
$4.00 recorder for grade 3 for music (4th grade if new student to Waltham)
$50 optional band fee for students, 4th-8th,
$50 optional activity fee for students, 5th-8th, participating in sports or Scholastic Bowl,
$58.80 optional fee for milk,
$15.60 optional per yearbook
We ask that all fees be paid in full at this time. Payment plans will be put into place for all families that do not fulfill this request.
PTCC fees must be paid separately.
Activity Fee (Sports and Scholastic Bowl)
Students in 5th-8th grade that will be participating in athletics (baseball, basketball, bowling, cheer, golf, softball, track, volleyball) will need a sports physical, proof of health insurance, a copy of the completed participation agreement, and the $50 sports fee paid at this time. Students on the Scholastic Bowl will only need the participation agreement and $50 fee paid at this time.
Junior High PE Uniforms
Students in grades 6th, 7th, and 8th may order a P.E. uniform from the school, or
Students may wear a red or black t-shirt and red or black shorts that are designated a P.E. uniform.
This year the P.E. order form will be part of our online registration. You will be able to order and pay for the uniform online or you can pay by cash or check. Please make checks payable to Waltham School. All orders are due by August 3rd.
PTCC
There will be sign-up sheets for room parents and committees.
PTCC fees include:
$21 hot lunch fee per student (monthly hot lunch),
$5 per student party fee
These fees need to be paid separate from the Waltham registration fees.
Students Entering Kindergarten
Students entering kindergarten will be required to provide a health physical exam, dental exam, vision exam, up-to-date immunization record, and a certified copy of a birth certificate.
Health Examinations
Students entering kindergarten and 6th grade will be required to provide current health examinations prior to the first day of school. Any student that has moved into the district from out of state needs to present an Illinois State physical within 30 days of entrance into the district.
Dental Examinations
Dental examinations are required for all students entering kindergarten, 2nd grade, and 6th grade. Exams are required by May 15th of the current school year of an exam that has taken place within the past 18 months. Students who fail to present proof of a dental exam will be unable to register for the following school year.
Vision Examinations
All students entering kindergarten, or Waltham School for the first time, must present proof of vision exam by October 15th of the current school year of an exam performed within one year. Students who fail to present proof of an eye examination by October 15th will be unable to attend school until requirements are met.
Sports Physicals
A current certificate of physical fitness must be completed on or before the first athletic practice. Until all athletic requirements are met, students will be unable to participate.
Immunizations
Students entering 5th grade are required to have the hepatitis B series completed by the first day of school. Students entering 6th grade are required to have the Tdap vaccine, meningitis, and a diabetes screening. In addition to other vaccines, students entering kindergarten are required to have two doses of the varicella vaccine or a physician's confirmation of having had the chicken pox, and a diabetes screening.
School Supply Drop-Off / Meet the Teacher Night
Waltham’s school supply drop-off and Meet the Teacher Night will be Monday, August 15th from 3:30 p.m. – 5:30 p.m. This will be for all grade levels, K-8. Please use this time to drop-off your child/children’s school supplies to their classroom and get acquainted with their teacher prior to the first day of school.
Parents will be able to sign-up for the time/day of their choice for parent/teacher conferences at this time. You may also drop off student physicals, dental or vision exams to the office.

Car Riders
Early Release Days for Students
Students will be dismissed at 11:45 a.m. on the following days:
- Wednesday-September 21st,
- Wednesday- October 19th,
- Wednesday- January 18th,
- Wednesday-February 15th,
- Wednesday-March 15th, and
- Wednesday-April 19th.
Parent/Teacher Conferences
Parent/Teacher conferences will be held this year on Wednesday, November 16th from 2:30-7:00 p.m. and Thursday, November 17th from 2:30-7:00 p.m. Parents will be able to sign-up for the time and day of their choice during Supply Drop/Meet the Teacher Night on August 15th.
School will be dismissed early on November 16th and November 17th at 1:45 p.m.
Non-Prescription Medicines
Tardiness / Early Departure
Students will be marked tardy if they are not in their classrooms, ready for learning, at 8:25 a.m.
If your child needs to be picked up early, please pull up to the front of the building and call the office at 815-667-4417. We will sign your child out and send them out to your car.
No students will be dismissed between 3 – 3:20 p.m. unless they are leaving for a Waltham-sponsored event.
We ask that you please make every effort to make appointments for your children before or after school to minimize disruptions to the learning environment and office.
Dismissal
Unless written notice or a phone call from a parent/guardian is received by the school office one hour prior to dismissal, students will be sent home on the bus.
Report Cards
Junior High Report Cards
Report cards will report trimester and final grades. The criteria for earning honor roll status for the Awards Day Assembly will be based on the student’s cumulative GPA for the 3 trimesters. Requirements for honor roll are as follows: Honor Roll – 2.7-3.69 and High Honor Roll – 3.70-4.3.

Awards Day Assembly
The Awards Day Assembly will take place during the school day, prior to graduation. (We want all of our students to have the opportunity to participate and be recognized by their peers.) Further information regarding the assembly and confirmed date will be sent home after the March Board of Education meeting.
