2. Student Absence Report Form - To ensure that a child's absence is marked correctly, a parent/guardian must complete an absence report form within three school days of the date of absence. A new form should be completed each day your child is absent. This form should be completed for IR students ONLY.

Annual health history forms are now on-line! If you haven’t completed this important document for the nurses, watch for an email notification from the district. Questions? Contact the Indian Rock nurses at 717-885-1250 option 4.

COVID protocol and absences should follow the recommendations of the District, which can be found at https://www.yssd.org/coronavirus-updates/. An absence report should be documented for the day(s) the student is absent awaiting a COVID test result or isolating for 5 days after a positive test result, with mask-wearing for days 6-10. A doctor's note is not needed to document COVID illnesses.
