Vision Screening Information
Parents/guardians that do not want their student to be screened at school must complete and turn in this opt-out form. Vision Opt-Out Form
PTO Spirit Night & Fundraiser
What to expect on picture day
- When an order is placed online, the parent/guardian will receive a camera pass via email. This can either be printed or texted to the student and brought to picture day. The photographer will photograph the printed camera pass along with the student.
- If a student is not able to print their camera pass, that is not a problem. We will match up the student to their order manually in the production process.
- A parent may order from the flyer up until 24 hours after picture day. After 24 hours, the online ordering access may close. This will allow our imaging team to "Pull" all orders submitted and match them with the students photographed.
- When pictures are returned to school, all students that purchased pictures will receive their prepaid package. Any student that was photographed but did not order a package, will receive a "Proof" (PX) of their picture that was taken on picture day.
Spirit Wear
PTO is selling spirit wear! This can be worn as part of your uniform any day of the week. Ordering closes Sept 1 and apparel will be delivered to your student after our last bulk order. Thank you for supporting the PTO!! Click the button below to order!

Practice times
Drill Team: Thursdays from 3:00 - 4:00 pm at Prov Hall Elementary gym (new gym) - The team meets at 2:45 in the portable and walks together to the gym.
Air Rifle Marksmanship Practice Times:
--Saturday mornings from 9:00 - 10:30 am in the cafeteria
--Tuesday afternoons after school from 3:00 - 4:00 pm in the cafeteria
Meet at the JROTC storage shed (south side of the building).
Color Guard: Tuesdays from 3:00 - 4:00 pm at the high school (JROTC shed (south side of building).
Orienteering Meets
What: Score Orienteering and Relay Orienteering Competition sponsored by Herriman HS Army JROTC
When: Sat, 07 Oct 2023 from 0900 (9am) - 1300 (1 pm). Leave PHHS portable trailer at 0830.
Where: Yellow Fork Canyon, 8000 Yellow Fork Canyon Rd, Herriman, UT 84096
Two-person teams will compete for 1st - 5th place medals in score orienteering and 1st - 3rd place medals in the relay event
What: Utah Orienteering Club Meet - Big Cottonwood Regional Park (Holladay Spook-O)
When: Fri, 20 Oct 2023 - 2000 (8 pm) to 2100 (9 pm). Registration begins at 1930 (7:30 pm), mass start at 2000 (8 pm) - 60 min time limit
Where: Cottonwood Regional Park, 4300 S 1300 E, SLC, UT Meet at the parking lot off 1590 E Road, west of Holladay Lions Rec Center.
There will be assigned "monsters" to chase and capture the punch cards of others (more instructions at registration). Bring headlamps, flashlights, or glow sticks to help you navigate the park and its hidden obstacles at night. Great activity for teenagers looking for an alternative to the traditional Halloween Party. Costumes encouraged!!
Physical Fitness Competitions
What: Northridge High School Monster Mash
When: Saturday, 28 Oct 7:30 am - 2:00 pm. Meet at PHHS portable trailer at 6:15 am.
Where: Northridge High School, 2430 North Hill Field Road, Layton, UT 84041. Transportation provided. Leave at 6:30 am.
What: Dixie High School Obstacle Course Challenge
When: Sat 18 Nov
Where: Dixie High School, 350 E 700 S, St George, UT 84041. Transportation provided. Bus leaves at 0500.
Coca-Cola Scholars Program
The Coca-Cola Scholars Program scholarship is an achievement-based scholarship awarded to students in their final year of high school. Students are recognized for their capacity to lead and serve, as well as their commitment to making a significant impact on their schools and communities. 50 Coca-Cola Scholars are selected each year to receive this $20,000 scholarship. The 2024 application will be available August 1-October 2, 2023, for students who will graduate during the 2023-2024 academic year.

Make Your Mark Contest
Students can create an original bookmark for the 2023 my529 Make Your Mark contest for a chance at a $1,000 my529 college savings scholarship account from my529 and the Utah System of Higher Education. Utah students enrolled in kindergarten through 12th grade are eligible to take part.

Hercules First Federal Credit Union Scholarship Contest
Open to high school seniors who are planning to pursue an undergraduate degree in 2024. Students must submit a 750-1000 word essay that ask about the intersection of financial literacy and social media (see attachment below for full prompts).

Elks National Foundation Most Valuable Student Scholarship
The 2024 application opened on August 1, 2023. Applicants will be judged on scholarship, leadership, and financial need. Current high school seniors, or the equivalent, who are citizens of the United States are eligible to apply.

Fall 2023 Jumpstart Coalition Scholarship
Up to three applicants will each receive a $1,250 scholarship award, AND one eligible applicant will receive an additional $500 bonus to their scholarship from my529 totaling $1,750. Students must be juniors or seniors, and have successfully completed, or currently be enrolled, in the General Financial Literacy course. Applicants must make a 60-second YouTube video that answers the question, “What are 3 important skills from your financial literacy class that will help you prepare for college and life after college?”

This program is open to all high school seniors planning on a technical or vocational education or students with special medical or financial needs.

Department of Defense Science, Mathematics, and Research for Transformation (SMART) Scholarship
The SMART Scholarship-for-Service Program is a combined educational and workforce development opportunity for STEM students. SMART offers scholarships for undergraduate, master's, and doctoral students pursuing a STEM degree. Scholarship recipients receive full tuition, annual stipends, internships, and guaranteed employment with the Department of Defense after graduation.
**This scholarship is for students who have already started college and are 18+ years old, but if you are interested in pursuing a STEM career, review the eligibility requirements now and keep this scholarship in mind for the future!
