Child Outcome Summary SPP 7 Reminders
1. A new student. Summarize the assessment data related to the child and record the data on the COSF within 30 school days after a child (age three, four or five) has been found eligible and placed in the Preschool Programs for Children with Disabilities Program (PPCD).
2. A transfer student from another district. Summarize assessment data related to the child and record the data on the COSF within 30 school days after a child (age three, four or five) has transferred into Special Education from another district.
Note: For a transfer student who has existing Entry Data and moves between campuses in the same district, “new” Entry Data are not required to be reported on the student by the receiving campus.
3. A student with an auditory and/or visual impairment enrolled in a school receiving special education services prior to age three. Upon the child’s enrollment in a PPCD program at age three, an assessment team will complete the Child Outcomes Summary Form (COSF) using current assessment data reflecting the child’s current functioning levels as he or she prepares to enter the PPCD program. The assessment date used will be the date the assessment team meets and completes the COSF.

This online learning module provides key information about the COS process and the practices that contribute to consistent and meaningful COS decision making. Participants will learn about the following topics:
- Why child outcomes data are collected
- The key features of the COS process
- The essential knowledge needed to complete the COS process
- How the three child outcomes are measured through the process
- How to identify accurate COS ratings using a team-based process
- The importance of comparing children’s current functional performance to age-expected functioning
- When and how to measure progress in the three child outcome areas
- How to document ratings and evidence to support those ratings in COS documentation

The Autism Circuit is a website dedicated to providing educators with evidence-based tools to support students with autism and other developmental disabilities. The toolkit on the website is sorted into four categories: Behavior, Communication, Social, and Classroom Organization. For every tool you will find:
- An explanation of how and when to use it
- Printable templates and resources
- Links to opportunities to further your knowledge on the evidence base for that strategy
