Information for Parents
Student COVID Screening Testing Program
The Sherwood School District is implementing a voluntary COVID screening testing program for families who wish to participate. Oregon Health and Science University (OHSU) is the local testing lab and facilitates the process. Parents/guardians who wish to participate will be provided with the materials to test their child weekly using a self-administered, at-home test. Results are shared directly with parents/guardians by OHSU. Our District’s role is to sign up families, send home testing kits with students for parents/guardians to administer the test in their home, and collect the tests weekly at the school location for pick up by OHSU. SSD is providing access to the OHSU program in response to feedback from families in our district requesting to participate in student screening testing.
The sign up process requires families to complete the attached OHA Consent Form and OHSU Authorization Form (for students over the age of 18, please complete this version of the Authorization form instead). Please submit completed forms to the main office at your child’s school, or contact the office if you need printed versions or forms in a language other than English.
Once a family has completed the sign up process, OHSU will provide our schools with testing materials, which schools will distribute to participating families. Each Friday morning, the parent/guardian should administer the test to their child. Completed tests will then be dropped off at the school that morning and picked up by OHSU for evaluation. Test results are shared directly with the parent/guardian. A positive test result will prompt a call from OHSU to the parent/guardian with further information. In accordance with our current COVID response protocols, the District asks that parents/guardians contact the school office to report any positive student test results.

Parents/guardians that sign up will receive additional instructions in the coming weeks. We are finalizing the timeline for program implementation with OHSU, and while the start date for testing is not yet confirmed, we expect this program to begin after the Thanksgiving holiday.
If interested, please return your completed OHA Consent Form and OHSU Authorization Form to your school office by November 12, 2021.

The Sherwood Education Foundation (SEF) was formed to enrich and enhance the quality of education in the Sherwood School District by distributing funds through grants to individual teachers, and by funding programs that support the district’s vision for educational excellence. Because of community support, in the last five years alone SEF has awarded 98 teacher grants totaling over $220,000. Whether it's Sensory Paths, Ozbot Evo Robots, Spanish Fiction Books, a Forensic Microscope, or Drumline Replacement for the High School Marching Band, Sherwood Education Foundation is committed to being there for our teachers and their students.
