The required grade specific dental examinations are due to the building health office for kindergarten, second, and sixth grade students by May 15, 2023.
If your student was referred for further evaluation from the vision or hearing screen at your student's school, please get the examinations completed as soon as possible and turn in the evaluation form to the health office in a timely manner. Students learn best when they can see and hear the material presented to them!
For Fall 2023 enrollment, please take note of the following requirements and make your appointments as soon as you are able in order to avoid school exclusion in the fall. Kindergarten and sixth grade students will need a physical exam and required immunizations to be completed and submitted by October 15, 2023. Kindergarten students will need a vision examination to be completed and submitted by October 15, 2023. Kindergarten, second grade, and sixth grade students will need a dental exam completed and submitted by May 15. 2024. Specifics and forms for these exams are available on the school website or at your doctor's office.
