Attention 6th Grade Parents
California Healthy Kids Survey
Dear 6th Grade Parents,
Your child is being asked to be a part of our school’s California Healthy Kids Survey (CHKS), sponsored by the California Department of Education (CDE). This is a very important survey that will help promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence. Your child does not have to take the survey. Participation is voluntary and requires your permission.
Survey Content. The survey gathers information on developmental supports provided to youth; school connectedness and barriers to learning; school safety; health-related concerns such as physical activity and nutritional habits; alcohol, tobacco, and other drug use; risk of depression and suicide; and protected class identifiers such as sexual orientation and gender identity.
You may examine the questionnaire in the school office or at your district’s website www.antiochschools.net.
The results from this survey are compiled into district- and county-level CHKS Reports. To view a copy of your district’s report, go to https://calschls.org/reports-data/search-lea-reports/ (Outside Source) and type in the district name.
It is Voluntary. Students who, with your permission, agree to participate do not have to answer any questions they do not want to answer, and may stop taking the survey at any time.
It is Anonymous. No names are recorded or attached to the survey forms or data. The results will be made available for analysis only under strict confidentiality controls.
Administration. The survey will be administered on February 20th – March 22nd.
Potential Risks. There are no known risks of physical harm to your child. Risks of psychological or social harm are very small. None have been reported in 22 years of survey administration. In rare instances, some discomfort might be experienced from the questions. The school’s counseling services will be available to answer any personal questions that may materialize.
For Further Information. The survey was developed for the CDE by WestEd, a public, non-profit educational institution. If you have any questions about this survey, or about your rights, call the district at 925-779-7500*40524 Essence J. Phillips.
Permission Slips will be coming home soon for all Jack London 6th graders. Thank you!

Other Procedure Reminders:
BICYCLES, SCOOTERS, SKATEBOARDS, AND WHEELIES
Bicycle racks are available for students who ride bicycles to school. To prevent bicycle thefts, parents/guardians should make sure children have and use bicycle locks. Scooters are acceptable but securing them can be difficult. Students are to walk their bikes/scooters while on campus, including the sidewalk areas. Please discuss safety with your child before he/she is allowed to ride to school, including the importance of wearing a bicycle helmet (California State Law). The school is not responsible for damaged or stolen bicycles/scooters. PLEASE NOTE: Roller blades, wheelie shoes, and skateboards are not allowed on campus.
BIRTHDAYS
Sometimes parents/guardians like to provide a special treat for their child’s classmates on their birthday. If doing so, please follow these guidelines:
- Please communicate your plans with the classroom teacher in advance.
- Send all items with your student in the morning, to reduce interruptions to the classroom. Please note that balloons, flowers, presents, etc., are not allowed in the classroom as they cause distractions to our learning environment.
- Please note that class time will not be taken for birthday celebrations, and treats will likely be given out at lunch or at the end of the school day.
- We strongly recommend a nutritional snack or even small trinkets such as pencils, erasers, or stickers rather than food since so many children have food allergies. This also promotes healthy habits for students.
- All edible treats must be store bought and individually packaged. Please do not send cakes or ice cream, which require cutting/preparation. All items should be easy for the teacher to hand out.
- Please provide the same item/treat for every child in the classroom.
- Please do not send party invitations to school (for birthday parties at home) unless the entire class is included.
- If your child does not celebrate birthdays or your family follows specific cultural traditions, please speak with your child’s teacher as early in the year as possible so these traditions can be honored.
- If your child follows a special diet, you might consider leaving acceptable treats with the teacher at the beginning of the year, so that your child can still celebrate special days with treats he/she can eat safely.
- Please contact your child’s teacher with specific questions.
CELL PHONE AND SMART WATCH POLICY
We realize that many students have cell phones. Cell phones can quickly become an issue/distraction at school. Please be aware of and review these cell phone rules with your child.
Cell phones may be brought to school as a safety precaution for use to and from school only. During the academic day, all cell phones must be turned OFF and kept in the student’s backpack. All smart watches must be in watch-only mode or off and in backpacks. Exception: A student may receive permission from staff to use a cell phone, and it must be used in the presence of school staff. The school and staff are not responsible for damage to or loss of cell phones or smart watches. Text messaging, taking videos or pictures, or abuse of cell phones is forbidden at all times. Students who violate these policies/rules will have progressive consequences (see below).
Consequences for Violation of Cell Phone/Smart Watch Policy:
1. Warning and review of policy.
2. Phone confiscated for the day (student picks up phone at dismissal) and parent phone call.
3. Phone confiscated for the day (parent picks up phone) and parent phone call.
4. Phone banned from school OR check in/out from office each day.
DOGS / PETS ON CAMPUS
Dogs and other animals are not allowed on campus at any time. Specifically, according to the state’s education code, dogs are prohibited from school campuses, whether or not the dogs are leashed. This includes school events. We have several students who are either afraid of, or allergic to, certain types of pets. This policy keeps our campus safe and friendly. If families are including a dog walk at the same time they are picking up or dropping off their child, we request that you meet or say goodbye off campus. If you notice a dog on campus, leashed or not, please inform the school office. It is our intention to keep our campus safe. Service animals are exempt from this state law. If you are to bring a service dog on campus, we must have paperwork on file in the front office.
DRINK POLICY
Due to ongoing issues with vandalism and disruption, certain drinks such as Gatorade, sports drinks, sodas and the like, are not permitted. We try to promote healthy habits and also protect our furniture and carpet from being stained due to spills. Drink sharing is not permitted.
Jack London Drink Policy:
· In class – plain water only from a water bottle or the drinking fountain.
· At lunch – 100% fruit juice, milk, and water only. Capri-suns are okay for lunch.
· No sugary, dyed, caffeinated drinks, energy drinks, or drink enhancers are permitted at school.
SCHOOL VISITORS
All visitors are required to report to the school office upon arriving at campus. Classroom visits must be arranged in advance and have prior approval from the classroom teacher. Parent/guardian volunteers sign in and receive their volunteer badges in the office. Please avoid lengthy conferences with the teacher during such visits so the teacher can conduct class as usual. Students from other schools, or siblings of Jack London students, will not be permitted to visit during the regular school day.
Please note that volunteers/visitors may have their visiting privileges limited or revoked if the classroom teacher, principal, or principal’s designee determines that the individual’s presence in the classroom, or on campus, obstructs, interferes, or disrupts the classroom or school environment.
SCHOOL VOLUNTEERS
Many enriching and worthwhile experiences are afforded our children through volunteer help from parents/guardians. With this support, teachers are able to provide more direct instruction. Parents/guardians interested in volunteering time and services are requested to contact their child’s teacher. We encourage our parents/guardians to visit and volunteer in classrooms; however, please leave young children/siblings at home as they are a distraction to students in the classroom. Parents/guardians are not expected to discipline students as that is the job of the staff member. All volunteers sign a Volunteer Code of Ethics before working in a class or with students.
Please note that volunteers/visitors may have their visiting privileges limited or revoked if the classroom teacher, principal, or principal’s designee determines that the individual’s presence in the classroom, or on campus, obstructs, interferes, or disrupts the classroom or school environment.
