Things You Will Need When You Fill Out the Application
- Guardian ID
- Birth Certificate
- Residency Documents (Recent utility bill; lease/deed/mortgage statement)
- Social Security Card or State S Number
- Immunization Records
- Current Grades (report card and/or transcript)
- Previous STAAR results
