Pfizer Vaccine Available through Rogue Community Health Center
The Ashland School District is partnering with Rogue Community Health Center to provide an opportunity for free vaccination for students 12 and up. Students under 15 will need written parent permission for vaccination.

Ashland Student Vaccination Clinic
Clinic will be held in the Commons. Vaccinations must be signed up for in advance. We have ordered 36 doses for this clinic and they will be available on a first come, first served basis. Students will need to bring a completed Coronavirus Vaccination Screening Form, see attachment below. Also attached is the Fact Sheet for the Covid vaccine that we will be administering to students.
Students will need to plan to wait for 15 minutes following the vaccination.
The Pfizer vaccine is a two dose vaccination. The second dose clinic is scheduled for October 21st from 3-6 PM in the same location.
