Health Forms and Requirements

The State of Illinois requires certain documentation in order for your student to be eligible to participate in school. There are health requirements for Pre-K, Kindergarten, 2nd, 6th, 9th, and 12th grade students as well as any students entering an Illinois school for the first time.
