A message from Mrs. Austenberg
Greetings from the Health Office!
I hope that your family has enjoyed time spent together with extended family and friends this summer!
Thank you for continuing to drop off updated immunization records (grade 6), sports physicals with related paperwork and all pertinent health forms. PLEASE DO NOT EMAIL PHOTOGRAPHS OF ANY HEALTH FORMS. I WILL ONLY ACCEPT SCANNED COPIES.
ASTHMA/ALLERGIES/MEDICATIONS
An Asthma Action Plan (signed by parent and MD) must be submitted to the Health Office for students with asthma even if they have MD permission to self-carry their MDI. Please ensure your child has their MDI in their backpack daily if it is not kept in my office.
Allergy Action Plans and corresponding paperwork were sent home in June for students with known allergies; please return all completed forms in the provided bag.
Parents may drop off medications and Epipens at the main office ONLY WITH ALL REQUIRED SIGNED AND STAMPED MEDICATION FORMS prior to August 20.
I will be in my office beginning August 22.
Signed and stamped Prescription and OTC medication forms must be submitted to the Health Office in order for me to administer medication at school. Please do not send your child to school with ANY medications in their backpacks to be taken during the day as this is against Administrative Code.
ATHLETICS
All sports physicals and related paperwork are due on August 24. It takes an enormous amount of time to prepare for Dr. Porter to clear them on September 1. If there is an extenuating circumstance, please let me know ASAP.
Please refer to my Student Athlete Information page for instructions and links to all forms. Please submit ALL forms together.
Please contact me with any questions or concerns you may have via email since I am not yet in my office to receive phone calls. I encourage you to inform me of any health-related concerns regarding your child. Thank you and I look forward to partnering with you in caring for your children during the year.
Mrs. Austenberg RN, CSN-NJ
