5 Essentials Survey

Dear Evergreen Parent/Guardian,

The 5Essentials Survey will be open from January 24-March 31 2023. We are asking teachers, parents, and 4th – 8th grade students to fill out the survey online. 4th grade students will complete the survey in their classroom with their teacher. This survey is a comprehensive, evidence-based tool designed to drive improvement in schools. The survey will generate a detailed picture of the inner workings of our school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school. The survey takes about 10 minutes to complete.

The Illinois 5Essentials Survey is administered online by UChicago Impact at the University of Chicago, on behalf of the Illinois School Board of Education. This survey identifies five indicators that can lead to important student outcomes, including improved attendance and larger test score gains. The five indicators that affect school success are:

· Effective Leaders

· Collaborative Teachers

· Involved Families

· Supportive Environments

· Ambitious Instruction


We need at least 20% of Evergreen parents and 20% of Benjamin parents to complete the parent survey to generate a parent report for each school. If you have students at both schools, please complete a survey for Benjamin and Evergreen. If you have more than one child and they attend the same school you may participate multiple times. Only one parent per household can participate.

Your participation in the parent portion of the survey will help us understand the conditions at your child’s school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.

Note: Parents have the option to leave their email address so that we can send them confirmation of survey completion upon submission of the parent survey supplement. That text box can be left blank if the parent doesn’t want to provide or have an email address. Thanks in advance for your participation.

Snow Gear

In order for a child to play in the snow at recess, please send them to school with snow pants, boots, a winter coat, a hat, and gloves. If your child is in need of any cold weather clothing items, please contact Mrs. Smith.

Upcoming Assembly Event: February 13th

Candy Bar Bingo

Evergreen hosted the PTA sponsored Candy Bar Bingo event. We had an amazing turn out and needed to find more chairs and tables than we originally planned for to accommodate all of the participants! Thank you to the PTA for sponsoring such a fun event for the families of Benjamin 25!

ACCESS Testing

Students that qualify for English language Services will be taking the ACCESS assessment beginning the week of January 23rd. This assessment is a mandated assessment for all students that qualify for English language services. This assessment assesses a students understanding and usage of the English language.

STAR & FastBridge Assessments

In the next couple of weeks, students will be participating in STAR testing in reading and math. This assessment is a computerized assessment that is given three times a year to all students in kindergarten through 4th grade. In addition, students will be assessed on their reading fluency progress through FastBridge. The Evergreen staff uses the information gathered on these assessments to plan instruction for their students.

Birthday Invitations: PTA Member Hub

There have been many of you asking about how to deliver birthday invitations at school. Your child can deliver birthday invitations at school if they are for everyone in the class or for every gender in the class (all the boys or all the girls). If you are only inviting select members of the class, please refer to the PTA Member Hub. You may be able to find contact information for students if they are registered through the Hub.

Junior Achievement Volunteers Needed

We are looking for parent volunteers to come in and provide Junior Achievement lessons to the students in Kindergarten-4th grade. Junior Achievement is a wonderful program that is dedicated to educating students about entrepreneurship, work readiness and financial literacy through hands-on and engaging lessons. The lessons are well planned and easy to implement. If you are interested in volunteering for this program or have any questions about it, please contact the PTA coordinator, Shea Wanshek.

Need the Newsletter Translated?

If your child is going to be absent from school for any reason, please call the Evergreen Office, 630-876-7810, to report your child's absence no later than 8:00 AM the day of the absence.
