Please be sure to turn in the required health documents for your student's grade. Students who do not submit the required documents may be subject to school exclusion. Kindergarten students need a physical exam and required immunizations to be completed and submitted by October 15, 2023, or they are subject to exclusion. Kindergarten students need a vision examination to be completed and submitted by October 15, 2023, or their report card may be held. Kindergarten and second grade students need a dental exam completed and submitted by May 15, 2024, or their report card may be held. The exam forms are available on the school website or at your doctor's office.
There seems to have been a glitch with the computer program with some of the health forms when some families were registering. If the building nurse requests an updated form or if you receive an email requesting you to complete it again, please do so. We want to make sure that we have all of the current information for all of our students.
Dates for Evergreen's vision and hearing screening have been added to the school calendar for mandated grades, teacher referrals, parent referrals, or students receiving special education services. If your student wears glasses or hearing devices, please be sure to send them with your student each day. In lieu of the screening services required, a completed and signed report form, indicating the child had an ear examination by a physician and an audiological evaluation completed by an audiologist within the previous 12 months, is acceptable. The parent or legal guardian of a student may object to hearing or vision screening tests for their children on religious grounds. If a religious objection is made, a written and signed statement from the parent or legal guardian detailing such objections must be presented to the local school authority. Screening instruments, test procedures and referral criteria are defined in the Illinois Administrative Code. Children whose test results meet referral criteria are referred to an eye doctor or family physician for further evaluation.

Benchmark Assessments
During the month of September, Benjamin School District 25 uses the STAR Enterprise Assessment and Fast Bridge to determine instructional needs and address individual learning across the district.
STAR Enterprise is a computerized classroom assessment which will be used to build a complete picture of your child’s mastery of skills in reading and math, as compared to other students and the Illinois Learning Standards, across the country. This assessment employs computer adaptive testing which greatly reduces time required for testing and makes instructional time more productive by providing accurate information about student achievement, growth and instructional needs.
Fast Bridge CBMreading is used to identify the level of student’s progress in reading competency and fluency. Students read aloud a grade level passage for one minute.

Curriculum Night for 2nd, 3rd, and 4th Grade
Please join us for the curriculum night next week. This night is for parents/guardians to learn from classroom teachers about classroom procedures, routines, expectations and curriculum.
The Principal Presentation on both nights will be in the main gym and will review our SOAR expectations, arrival/dismissal procedures, safety drills, and communication pieces. (The presentation is the same as the one on the night of August 31st.) Parking will be limited on, so please walk or carpool if possible.
Open House Night for 2nd-4th grade will be September 7th.
