This newsletter will have information helpful for you to understand what you need to do to start accommodations for your college courses you are taking while in high school.
I am the coordinator for disability services at the college. I am happy to help you get your accommodations in place for your concurrent educational pursuits at ASU-Beebe. STEPS FOR CONCURRENT ACCOMMODATION REQUESTS
Concurrent students must complete the accommodation steps below so that the Office of Disability Services can create ASUB Accommodation Notice for college instructors. Concurrent Students IEP/504 plans govern the high school classes but do not apply towards ASUB college courses.
STEP 1: Complete and turn in the Concurrent Accommodations Request Form to Office of Disability Services. (Provided in this newsletter.)
STEP 2: Provide high school accommodation documents such as IEP/504 Plan, or other disability type documents, if student is home schooled. Some accommodations provided for high school classes will not be provided at college, such as modified testing and para support individuals.
STEP 3: Office of Disability Services reviews the request and supporting IEP/504 or disability documents. If all information provided is complete, the office issues the ASUB Accommodation Notice to students' instructors and students via ASUB email.
STEP 4: Accommodations start at the point of email notification to instructors. Instructors are notified of student's accommodations via the Accommodation Notice.
STEP 5: Student works out details of each accommodation with each instructor to confirm all arrangements are in place for the semester for all courses. The Office of Disability Services is available to assist as requested.
CONCURRENT ACCOMMODATIONS REQUEST FORM
This form is completed online and when submitted, goes straight to the Disability Services Coordinator. You will need to know what high school accommodations you are receiving and the name of your high school IEP/504 counselor is to complete this form.
Your high school IEP/504 counselor can assist you in completing the form.
DISABILITY DOCUMENTATION SHOULD INCLUDE
Disability Documents: High School IEP or 504 Records
If you have an IEP in high school, please request the following four documents.
1. Summary of Performance (if available)
2. IEP
3. Diagnostic Testing Report
4. Program Evaluation Conference Decision Form
If you have a 504 Plan in high school, please request the following two documents.
1. 504 Plan
2. Diagnostic Report
General Information:
Disability documents might come from a doctor's office, mental health clinic, testing report, or specialist report.
Your disability document must clearly have your name on it and clearly have identification as to whom has written the document such as doctor's/clinic's letterhead and signature.
Your disability document should include information regarding your diagnosis, treatment information, and any other important details regarding your diagnosis.
Documentation Guidelines:
1. Clear Diagnostic statement of the disability
2. How the disability impacts functioning
3. Documentation is provided by a licensed or otherwise properly credentialed professional
DISABILITY DOCUMENTATION-WHERE TO SEND
Your disability documentation can be emailed to the Coordinator of Disability Services, hand delivered to the Office of Disability Services or mailed to the Coordinator of Disability Services.

ACCOMMODATION NOTICES
WHAT IS AN ACCOMMODATION NOTICE?
The Accommodation Notice is a document that the office of disability services creates to list all of the approved college accommodations for the concurrent student. The Accommodation Notice is developed once the student has completed the accommodation steps.
IMPORTANT ACCOMMODATION NOTICE DETAILS:
Accommodation Notices must be issued through the Office of Disability Services for ASU Beebe courses. Concurrent Students must notify the Office of Disability Services each time an Accommodation Notice is needed for college courses taken at the the college.
Accommodation Notices are only issued through the office of disability services when the student has completed the above steps.
Accommodation Notices are the documents that notifies instructors of the student’s approved college accommodations and accommodations start at that point of notification.
Accommodation Notices are emailed to instructors from the Office of Disability Services.
