A message from our school nurse
Welcome back to a new year!
As a reminder, the parents of all new and returning students should log into PALCS School Health Portal at secure.magnushealthportal.com to update required health information for the 2022-2023 school year. If you don't have a password, click the link that says "I'm having trouble logging in" or contact me for a new registration link. When you have logged into Magnus, please answer the Conditional and Health Information Questions. If your child attends CPFA in person, please also fill out the Over-The-Counter-Medication section. This must be completed before school starts.
For grades that require a Physical, Annual Screening or Dental Exam, we will accept any forms dated in 2022. Height, weight, vision and hearing screenings will be done by the school nurses in the fall for students attending school on site.
If your child/children are NEW to PALCS this summer and are in the process of orientation, you may receive a message stating that we are missing your child's immunization record. The immunization record you supplied at registration will be uploaded by Health Services. This data is manually entered, and may take us a few weeks into the school year to complete.
Automated e-mail reminders are sent out monthly until all of the items have been completed.
The following items are due for each different grade level:
ALL GRADES: Health screenings will be offered on site at CPFA in the fall. If your child attends virtually, please complete the Annual Screening Form and upload into Magnus or contact me to schedule a screening at CPFA for your virtual student.
Grade 6: Physical Exam
Grade 7: Dental Exam and Immunization- Tdap and Meningitis. Update immunization or exemption before entering 7th grade. This is a required immunization for entry to 7th grade.
Grade 11: Physical Exam
Grade 12: Immunization- Meningitis. Update immunization or exemption before entering 12th grade. This is required immunization for entry to 12th grade.
Thank you!
