WPS Health Services Updates

The nurses have started their annual Health Screenings. If you want to know more about the health screenings, please click the picture below. If you have specific questions regarding your child's screening or to opt your child out, don't hesitate to get in touch with your child's school nurse directly.

Please bring your child's Epi-pen, Doctors Orders, and Parent Consent Form to your School Nurse. Please click on the picture above to access the Parent Consent Form.

If you have not turned in your child's Health History form, please print the form below and return it to your child's school nurse as soon as possible.
