GMS Falcon Update
8/20/23
Transportation Information
Don't forget to continue to check infofinder for live updates on transportation routes.
http://infofinderi.com/ifi/?cid=GLSD4QECLLXTS
Middle School Students will be able to ride their assigned bus from the HS to the MS as a shuttle in the mornings. Buses will leave the HS by 7:15 so students will need to be in the front of the school. It will be important that students have signed up for transportation to and from their place of residence so that they can be assigned a bus for organization and safety reasons. Even if they do not ride daily to and from their residence, we want to be organized and provide a safe route for all students. Please contact the transportation office with any questions - 937-663-4855
Please be sure to complete/update Online Forms
** Consider Final Forms updates like a "re-enrollment" that needs updated every year and throughout the year if there are changes to address or contact information. This is required for everyone **
Prior to the school year, you will need to make updates to your child's Emergency Medical and Transportation forms! This needs completed every year. If nothing has changed, you should only need to verify information.
Students will not be able to pick up their schedules if forms are not completed on the parent side. We will have computers available in the office.
Meal Account Information
Meal prices for Middle School and High School 23-24:

Immunizations for Grade 7
Ohio Law requires that all seventh graders must have received multiple immunizations. Before starting school, students in the 7th grade must have a Tdap (Tetanus, Diptheria, and Pertussis) and a Meningococcal vaccine. Written proof of receiving these vaccinations must be on file in the school office. Students who do not have evidence of proper immunizations will be excluded from school after 14 days, per Ohio Revised Code.
If your doctor says immunization might be harmful to your student, a waiver must be on file. If you object to immunization for good cause, for example religious convictions, a waiver must be on file.
If you need them to be faxed, our Fax number at GMS is 937-663-4674
