Health Office Update
Please be sure to turn in the required health documents for your student's grade. Students who do not submit the required documents will be excluded if mandatory paperwork is not submitted. Kindergarten and sixth grade students will need a physical exam and required immunizations to be completed and submitted by October 15, 2023, or they will be excluded on October 16, 2023. Kindergarten students will need a vision examination to be completed and submitted by October 15, 2023, or their report card may be held. Kindergarten, second grade, and sixth grade students will need a dental exam completed and submitted by May 15, 2024, or their report card may be held. The exam forms are available on the school website or at your doctor's office. Please contact your building nurse to discuss your situation if the forms will not be submitted by October 15.
This is also a reminder that no student should have any medication with them during school hours (including cough drops, eye drops, cold medication, cough medication, etc) unless they have the proper paperwork in place to administer it to themselves. An inhaler and Epi-pen are the only medications that can be carried with a student for self-administration at this time. Having students carry other medication could be a dangerous situation if the medication got into the wrong hands. Any parent is welcome to come to school to administer medication to their student as needed. Per Illinois School Code "the administration of medication to students during the regular school hours and during school-related activities should be discouraged unless absolutely necessary for the critical health and wellbeing of the student.” If this is the case, please complete the necessary forms to be completed by the physician and talk to the nurse at your student's school building to make necessary arrangements.
We are beginning to see cases of common illnesses and health problems at both buildings. So far we have heard about hand, foot, and mouth, strep, pink eye, the stomach bug, COVID, and lice all reported to the health office. Please be vigilant in checking your students for their wellness and continue to keep them home when ill. When home, it is recommended to consult your child's physician for treatment options and needs based on their symptoms. Please inform the building nurse of any concerning health conditions. Let's have a safe and healthy school year!
Dates for Evergreen's vision and hearing screening have been added to the school calendar for mandated grades, teacher referrals, parent referrals, or students receiving special education services. If your student wears glasses or hearing devices, please be sure to send them with your student each day. In lieu of the screening services required, a completed and signed report form, indicating the child had an ear examination by a physician and an audiological evaluation completed by an audiologist within the previous 12 months, is acceptable. The parent or legal guardian of a student may object to hearing or vision screening tests for their children on religious grounds. If a religious objection is made, a written and signed statement from the parent or legal guardian detailing such objections must be presented to the local school authority. Screening instruments, test procedures and referral criteria are defined in the Illinois Administrative Code. Children whose test results meet referral criteria are referred to an eye doctor or family physician for further evaluation.

Sneaker Drive
Did you know that your gently worn sneakers could help Benjamin/Evergreen PTA?
Well, they can and here’s how…Our school is participating in a fundraiser where there is nothing to buy or sell; All we need is your support and your gently worn, used, or new sneakers!
Here’s how you can help:
Start in your and your family’s closets and gather as many pair of sneakers that you no longer want; maybe you’ve out-grown them, they’re out of style, or out of season. If you and your household have 20 pairs, great! If not, don’t worry. You can ask your neighbors, friends, co-workers, and anyone in your social networks to help out. Every pair counts! Please bring the sneakers into Evergreen and put them in the yellow container by the main office. (The container outside is for a different purpose... more on that to come!)
The sneakers that we all collect will help raise money to support our program AND will help our
environment by keeping sneakers out of landfills.

Evergreen Fun Run
The Benjamin-Evergreen PTA is ready to host another Fun Run on October 20th full of exciting prizes! This year's top fundraiser wins their choice of a $125 gift card to Urban Air Adventure Park OR Nike.com! Your fundraising dollars will go towards a new laminator at Evergreen. The laminator is used to create learning materials and games for students. You have a month to Get Ready, Get Set, and Go! (More information will be sent home!)
