If you have a child who will be five on or before September 15, 2019 now is the time to complete Kindergarten Sign Up if you haven’t already done so.

Our building previously hosted Kindergarten Sign Up in April. If you missed this event please contact the Samuelson office as soon as possible. If you do not complete Kindergarten Sign Up at Samuelson prior to July 14, 2019, you will have to come to the Welcome Center to add your younger student before doing Online Registration for the 2019-2020 school year. Please bring the documentation below to sign up your child for kindergarten:

• Birth Certificate
• Immunization Record
• Photo ID
• Proof of Address which should be one of the following:
• Current utility bill or confirmation of services
• Current lease or purchase agreement
• Recent notarized letter from landlord or home owner
• Recent mortgage statement
• Polk County Assessors web page verifying ownership
• Recent property tax statement
• Recent vehicle registration
• Recent DHS paperwork

Online registration opens on Monday, July 15th. If you have a kindergarten student or have a change of address you will need to upload proof of your address in the online registration application. Proof of address may include:

Utility Bill or Confirmation of Services
Lease or notarized statement from landlord

Purchase Agreement
Mortgage Statement

Polk County Assessors- print copy of web page verifying property ownership
Property tax statement
Vehicle registration

DHS Paperwork

Shelter Stay Verification

We are in dire need of PTO volunteers. Samuelson PTO is involved with many activities such as Walk-A-Thon, Fun Night, Go The Distance Day, 5th Grade Party, and so much more! The more volunteers we have, the more opportunities there are for your child. Help us come together as a team and make the 2018-2019 school year the best one yet!

Samuelson t-shirts are still available. They are $10 and you can purchase them in the office.

Please remember to have your child read, write, and practice math facts daily. This will help them tremendously when returning back to school.
