Request for Tier 1 Learning Environment Change
Dear Parent(s)/Guardian(s),
For the next trimester at the elementary level, parents/guardians have the option of switching their child(ren)'s learning environment from virtual to in-person, or vice versa. Only families interested in switching their child's learning environment need to complete this form. Please review the COVID-19 Reentry Planning documents and Learning Plans on the district website (https://www.muskegonorway.org/about/mns-learning-plans.cfm) and consult as a family regarding the intent to switch your child(ren)'s learning environment prior to completing THIS FORM. By filling this form out, you will be emailed a survey retake link, within one business day, which allows you to retake the August 2020 survey with your new selection. The form and survey will need to be completed by NOON ON NOVEMBER 5, 2020. Again, there is no need to complete this form if parents are interested in having their elementary child(ren) remain in his/her current learning environment.
If you have a student in middle or high school, more information will be coming via those respective school's newsletters, later in November.
Thank you.
When and How to Notify MNS about COVID-19
PLEASE NOTIFY MNS IF ANY STUDENT OR STAFF MEMBER EXPERIENCES THE FOLLOWING SYMPTOMS OR STATUS USING THIS FORM.
The more you keep MNS informed the better we can
STAYSAFE to STAYOPEN.

Mill Valley Welcomes Mrs. Smith ~ Interventionist
Hello, my name is Carolyn Smith and I am thrilled to be one of the new interventionists at Mill Valley. I graduated from Illinois State University with a K-9 Elementary Education Degree. I have been teaching for 14+ years 4K-8th grade. I especially love teaching reading and math.
I grew up in the suburbs of Chicago. Some of my favorite things to do are traveling, kayaking, hiking, and swimming with my husband, son, twin girls and two dogs. My husband and I decided to move to Wisconsin in 2018 when our kids were grown up.
I can’t wait to get to know all the unique personalities of my students and discover how they learn best!
