Welcome to Office of Disability Services!
Thank you for reaching out! I am the coordinator for disability services at the college. I am happy to work with you to get your accommodations started for your educational pursuits at ASU-Beebe. I work with all students across all of the ASU-Beebe campuses (Beebe, Heber Springs, Searcy, LRAFB, Online, Zoom).
This newsletter will have information helpful for you to understand what you need to do to start accommodations.

How to Start Accommodations
STEP 1: Request for Accommodations Form
Complete the online Request for Accommodations Form to Office of Disability Services.
A paper copy can be requested from the office, if needed. Link to form is below.

STEP 2: Provide Disability Document(s)
Provide supporting disability document(s) to the office. Documents may be emailed to the office, may be dropped off in person to the office, or mailed to the office.

STEP 3: Meet with Office of Disability Services
Student meets with Office of Disability Services and develops the Accommodation Notice for the semester.
Appointments are available as a telephone, video conferencing, and Beebe campus location.

Request for Accommodations Form
This form is completed online and when submitted, goes straight to the Disability Services Coordinator.
There are no wrong answers, just complete to the best of your understanding. We will discuss all accommodation possibilities during the accommodation appointment.
Disability Documentation Should Include
General Information:
Disability documents might come from a doctor's office, mental health clinic, testing report, or specialist report.
Your disability document must clearly have your name on it and clearly have identification as to whom has written the document such as doctor's/clinic's letterhead and signature.
Your disability document should include information regarding your diagnosis, treatment information, and any other important details regarding your diagnosis.
Documentation Guidelines:
1. Clear Diagnostic statement of the disability
2. How the disability impacts functioning
3. Documentation is provided by a licensed or otherwise properly credentialed professional
Disability Documents: High School IEP or 504 Records
If you had IEP in high school, please request the following four documents.
1. Summary of Performance
2. IEP
3. Diagnostic Testing Report
4. Program Evaluation Conference Decision Form
If you had 504 Plan in high school, please request the following two documents.
1. 504 Plan
2. Diagnostic Report

ACCOMMODATION NOTICES
WHAT IS AN ACCOMMODATION NOTICE?
The Accommodation Notice is a document that the student and office of disability services creates to list all of the approved accommodations. The Accommodation Notice is developed once the student has completed the initial accommodation process.
IMPORTANT ACCOMMODATION NOTICE DETAILS:
Accommodation Notices are renewed each semester and may change depending on the classes for which the student is enrolled each semester. Accommodation Notices are not issued automatically. Each semester, students must request to continue accommodations.
Accommodation Notices are only issued through the office of disability services when the student has submitted the request for accommodations to the office.
Accommodation Notices are the documents that notifies instructors of the studentβs approved accommodations and accommodations start at that point of notification.
Accommodation Notices are emailed to instructors from the Office of Disability Services after the student and coordinator of disability services have met and agreed upon accommodations.
