The following are required documents for TELPAS Check-In:
PLEASE NOTE!! If forms listed are not used, they must be copied and included in your documentation and marked "NONE".
Please make sure that all documentation is in correct order and prepared before coming to the Testing Warehouse for TELPAS Check-In. Once all documentation has been approved at check-in, you will scan at the Testing Warehouse.
TELPAS ONLINE READING DOCUMENTATION:
- Room Traffic Forms**--All Seating Charts and Room Traffic Forms will be combined by day and test administrator. Ex. TELPAS Reading--Mrs. Jones Seating Chart/Room Traffic form "numbered" and combined together.

NEISD ATTENDANCE ACCOUNTING HANDBOOK FOR TESTING PROCEDURES
The following information was sent to campuses Friday, March 3rd from NEISD's Data Processing/MIS Department.
Below is the reminder from the NEISD Attendance Accounting Handbook for Testing Procedures for all campuses; Take note of the highlighted areas.
Responsibilities of all Secondary Attendance Clerks/Elementary Data Processors
Note: DO NOT post any “Y” attendance codes for students taking State Assessments.
1. Collect all rosters/seating charts from the testing classrooms/areas.
2. Post student absences using PPER.
3. Print an ALIS or APHN on every day a test is being administered
Elementary & Middle School - Period 2 High School – 2nd hour.
4. Record parent response on the ALIS or APHN printout as to why the student was not at school
(The Principal will work with the Testing Coordinator to appoint who will be responsible for recording the response on the form).
5. Compare the names of the students reported absent on the ALIS/APHN with the names of the students listed as “Absent” on the testing rosters/seating charts to ensure accuracy in attendance reporting for audit purposes.
6. Keep detailed documentation for EACH day of testing.
7. On plain sheet of paper indicate it was a day of testing and keep with daily attendance
documentation for audit purposes.

8. Give a copy of the documented ALIS or APHN to the Campus Testing Coordinator.
The Campus Testing Coordinator will use the ALIS/APHN only as a guideline on students that are absent in order to fill out the Testing Services Absence Report form, which is part of the Testing Administration documentation. See Example below:
Attendance Procedures for NON-Testing Students
Any students not testing and not in their regularly assigned classroom at the official attendance time will need their attendance taken.
1. A student roster should be created by a designated person on campus (Curriculum AP) and given to the teacher who is hosting these non-testing students.
2. If a whole grade level is not testing, an ATZ0001R Extracurricular Activity Attendance Report may be requested from the Data Processing Help Desk.
3. The hosting teacher will take attendance on the roster/ATZ0001R by indicating next to the student’s name “Absent” if the student is not present and “Present” if the student is present.
4. The teacher will sign, date and write their employee ID at the bottom of the roster.
5. The roster will be delivered to the attendance office by testing proctors.
These procedures are on pages 30 and 31 and are to be consistent throughout the district in order to be in compliance for TEA Audit. For information in regard to taking attendance, please contact the NEISD Data Processing/MIS Department.
