Physicals and Immunization Requirements Due October 15, 2022

Students must show proof of physical and immunization requirements by October 15,

2022 or be excluded from school.


  • Pre-K Requirements: Illinois School Physical and up-to-date immunizations
  • Kindergarten Requirements: Illinois School Physical, up-to-date immunizations, vision examination, and dental examination
  • Second Grade Requirements: Dental examination
  • Sixth Grade Requirements: Illinois School Physical, up-to-date immunizations and dental examination
  • Ninth Grade Requirements: Illinois School Physical, up-to-date immunizations, and dental examination
  • Twelfth Grade Requirements: Up-to-date immunizations, two doses of the MCV vaccine are required for 12th grade; however, only one dose is required if the first dose was received at 16 years of age or older.

Save the Date: Parent Teacher Conferences will be on October 20 and October 25th
