The ASQ (Ages & Stages Questionnaire)
Babies and toddles grow and change so quicky. Project Connect, with support from MA Dept. of Early Education and Care, can help you learn more about your child's overall development using a survey called "The Ages & Stages Questionnaire."
As parents, you are your child's first and most important teacher. We want to help you provide the best start for your child by giving you the opportunity to keep track of your child's development. Questionnaires can be provided every 2, 4 or 6 months and include questions about some things your child can and cannot do. The questionnaire asks about your child's communication, gross motor, fine motor, problem solving and personal social skills.
If you would like to receive the age-appropriate questionnaire. please contact one of the Parent Educators during a playgroups or home visit, scan the QR code provided here, or email Andrea, Project Connect Coordinator at awitherell@attleboroschools.com.
We will provide support for you and your family every step of the way!
