All students who participate in 6th-12th grade athletics and 8th-12th grade band are required to have an annual physical on file before they are allowed to participate each school year. This includes practices. Physical must be filled out on the OSSAA Physical Examination and Parental Consent form. CLICK HERE to download and print the form. Please turn in all physical forms to your respective site office.
