ANNUAL RESIDENCY VERIFICATION
FEE PAYMENT OR FEE WAIVER
Pay student fees using one of the following methods or request a fee waiver -
- ONLINE: Login to Skyward using the PARENT login and password and select Fee Management.
- MAIL-IN: Print this form and mail in with a check or money order post marked by July 30.
- IN-PERSON: Call 708-974-7470 to speak to the Cashier about an in-person payment option.
- WAIVER: If you believe you qualify for a fee waiver/free lunch for the 21-22 school year, complete and submit this form. You will receive follow-up communication if a waiver is approved or payment is required. See the Additional 21-22 School Year Resources section below for more detail on fee waivers. Families must submit a new Fee Waiver application with updated documentation every year.

Login to Skyward using the PARENT login and password and select 2021-2022 Consent.

HEALTH RECORDS
FRESHMAN/TRANSFER STUDENTS - PHYSICAL AND DENTAL
All incoming freshmen are required to submit a State of Illinois Certificate of Child Health Examination with proof of immunizations (Please complete and sign the Health History section) and a Dental Examination Form.
SENIOR CLASS OF 2022 - PROOF OF MENINGOCOCCAL VACCINE
All incoming seniors are required to submit proof of meningococcal vaccine on or after their 16th birthday.
Health documents are required to be submitted online through D230 Laserfiche https://www.d230.org/site/Default.aspx?PageID=1527 (No drop off or mail-ins will be accepted).

There are several options to get the vaccine before school begins:
- Come to Stagg for our Vaccine Clinic in partnership with Osco on Friday, July 30. Please register here. This is the last vaccine clinic scheduled before school begins. Please note that Osco does not take Medicaid or Meridian Health Insurance cards. There is a fee for those without insurance.
- Make an appointment with your regular health care provider.
- Visit your local Township. Here is the link for the Palos Township Health Clinic where vaccines are $10.
- Visit CVS, Walgreens, or Osco and ask if they give vaccines at that location.

July 30 (Friday) - Vaccine Clinic - Senior Class of 2022 - Register here for your required Meningitis vaccine.
August 13 (Friday) - 1st day for Freshmen and Transfer Students - No Zero Hour
August 16 (Monday) - 1st day for Sophomores, Juniors and Seniors
August 24 - Back to School Night for parents
Skyward Overview/Instructions
Fee Waiver - Application is Required Annually
Students currently eligible for Free Lunch, whose family is currently receiving aid under Article IV of the Illinois Public Aid Code or whose parent is serving in active duty or was injured or killed while serving active duty in any branch of the United States Military may qualify for a Fee Waiver.

The following income documentation is required as part of your electronic application -
- A current letter from the Department of Human Services on letterhead for SNAP or TANF benefits that includes a case number and names of all persons covered OR
- 2020 Income Tax Returns including corresponding W2 form(s)
Important Notes -
- The student(s) must be listed as a dependent on the 2020 Income Tax Return.
- Medical benefits only will NOT qualify for a Fee Waiver.
- Deadline for Fee Waiver applications after Schedule Release: 30 days from day 1 or 30 days from the transfer date.
- District 230 verifies information for 100% of those who apply for a Fee Waiver.
- If you qualified for a Fee Waiver/Free Lunch last year, you must re-apply for the new school year.
Free Lunch
District 230 uses the State of Illinois’ Direct Certification process to determine families who are automatically eligible for Free Lunch. These families are receiving assistance under the Supplemental Nutrition Assistance Program (SNAP) and/or the Temporary Assistance to Needy Families (TANF).
Families who are automatically eligible for Free Lunch will —
- Receive notification from the District prior to Schedule Release in August.
- Automatically receive a waiver of their student’s 21-22 school year fees.
Food Service - Add Money to Your Student's Account
To add funds to your student's Food Service account, go to stagg.d230.org and select Online Payments. Login to the D230 Webstore and select Skyward Food and Fees.
Transportation/Busing - Find your Route Number and Time
Login to Skyward Family Access to view your student's bus route. Click on "Busing" in the left list of selections. Your student's route will be listed next to AM Bus Number. In some instances a student may have a different AM and PM route. If this is the case, a route will be listed next to both AM Bus Number and PM Bus number.
To view route pick-up locations and times, click on "District Links" in the top right corner of your Skyward Family Access Account. Select Bus Routes - AAS and open the Regular Routes file. Scroll to your students bus route number within the document to view the available pick-up locations and times closest to your home.
On-Campus Student Parking - Class of 2022 Seniors Only
On-campus parking will be available for SENIOR STUDENTS ONLY. View the Senior Parking Requirement Information and Timeline for applications on the Dean's Office page of the Stagg website here - https://www.d230.org/domain/132. All eligible applicants will be entered into a lottery just prior to the start of the school year.
Athlete Registration
Athletes need to sign up for sports at stagg.8to18.com/accounts/login
Online registration opens in mid-July.
- A current physical (within the last 13 months) must be on file with the Athletic Office.
- Sophomore, Junior and Senior students should submit electronically to https://lf.d230.org/Forms/mxSUu.
- Freshman physicals should be submitted through the Health Office link only found here - https://www.d230.org/site/Default.aspx?PageID=1527
- You will receive an email confirming if the physical is approved or a resubmission is necessary with additional information.
- Please note the Athletic Office is closed starting June 21st. Sports physical submissions will be checked periodically after that date and follow-up communication will be sent at those times.
